Personal credit report application for all 25% or more stockholders and all officers
Business qualification application (credit report)
Approved warehouse line of credit (direct lenders only)
Errors & Omissions / Fidelity Bond Insurance binder (direct lenders only)
Photographs of facility
Floor plan of facility (can be hand-drawn)
Certification letter that applicant has not been denied a license, etc. (see letter below)
Quality Control Plan (must have a written quality control plan)
Copy of fictitious name filing approved (if applicable)
Copy of your Corporate license
Copy of your Articles of Incorporation
Application fee of $1,000 payable to HUD
